Office Hours

Our office hours are Monday – Friday 8:30 am – 4:30 pm

Appointments

Patients are seen by appointment only. We do not see walk-ins. If you need to be seen urgently please call our office at 865-483-3594.
Please be on time for your appointment. Late arrivals may be rescheduled.
Due to patients with severe breathing problems, please do not wear perfume/cologne.

Disc and report of CT, MRI, chest x-ray
Office notes from referring physician

Prescriptions

Prescriptions and refills are submitted electronically at time of appointment. If you need a refill at a time other than at an appointment, please call your pharmacy and have them submit a request to us. This will insure that electronic submission of your prescription is correct.

Referrals

Some insurance companies require a referral to a specialist.  If your insurance company requires a referral, please obtain one from your primary care physician prior to your appointment.

Health Insurance

Insurance information is required and documented at your first visit. If you make changes in your insurance, please let us know immediately. Co-pays are due at the time of service. Please be aware of your insurance plan’s policies regarding co-pay, referrals, etc.

HIPAA

The Health Insurance Portability and Accountability Act of 1997 (HIPPA) establishes the rules regarding medical records confidentiality. Each new patient must sign an acknowledgement of our policy and will be provided copy if requested.

HIPAA requires we obtain a patient signature before releasing medical records. If you wish to have your medical records released to anyone, please see our receptionist.
